This is help to fill out the Etransfer Contact Information form.
The first part of the form is Required fields.  This is personal information so that we can contact the buyer if there is any problems.
The second portion gives us the information on what was purchased.
    The Tickets line is the number and cost of tickets purchased.  If no tickets are purchased leave this line blank.
   The Tshirt line is the total number and cost of shirts to be purchased.  If no shirts are purchased leave this line blank.
   Grand Total is the total cost of the tickets and shirts purchased.
   The bottom box is for information of the shirt.
M/W = Mens or Womens
Type = Tshirt or Tank Top
Size = Small to XXXXL (refer to size chart below)
Colour = Black or Grey
Quantity = Quantity of this type of shirt.
